New Partnership Puts Evidence-Based Tools in the Hands of Primary Care Providers and their Patients
The Centre for Effective Practice and CognisantMD join forces to deliver EMR-integrated patient-facing forms at the point of care
Toronto, ON – November 12, 2015 – The Centre for Effective Practice (CEP) and CognisantMD are pleased to announce they are partnering to support the delivery of evidence-based programs and tools in clinical practice. This partnership will allow CEP to deliver its many evidence-based programs and tools through CognisantMD’s Ocean Platform on patient tablets, secure email, and data collected directly in Electronic Medical Records (EMRs).
An evidence-based clinical tool regarding poverty screening of patients is among the first topic area to be integrated into the platform and is available now to all Ocean users. Other tools include a lower-back pain screen and an opioid manager.
