Recent Additions to the Ocean Library
Are you wondering what’s new in Ocean? Our library of tools is continually growing, and now includes over 400 globally available (core) forms, plus an additional 1500+ forms in our shared library. To keep you up to date on the most recent updates, we’ve compiled a list of a few of the questionnaires and forms that were recently added or updated in the core Ocean Library.

Benzodiazepine Management Questionnaires
This is a series of tools to help in the management of benzodiazepine treatment. It includes an Ocean questionnaire to alert patients of their risk being on a benzodiazepine with a quiz to address misconceptions, an Ocean signed agreement between the patient and the physician (also called a benzodiazepine contract) to set expectations when safely prescribing sedative therapy, and a printable handout for patients motivated to self-taper their benzodiazepine (Not a patient-facing OCEAN form; this is a downloadable resource for printing).
FINDRISC Diabetes Calculator
Using Ocean, patients can now complete the FINDRISC screening for Type 2 diabetes risk, with the results automatically scored and entered into the patient chart.
Smoking Status – Expanded
The smoking status screen has been expanded to include all tobacco products. This screening tool is being used in many Ocean clinics to conduct universal smoking status screening. Using rules in Ocean, additional forms can be shown to the patient automatically based on their response. For example, a patient can be invited to a smoking cessation program, or to complete the Canadian Lung Health test. Contact us if you’d like to implement this in your clinic.
LACE Risk Score
The LACE index identifies patients that are at risk for readmission or death within thirty days of discharge. “L” stands for the length of stay of the index admission. “A” stands for the acuity of the admission. Specifically, if the patient is admitted through the Emergency Department vs. an elective admission. “C” stands for co-morbidities, incorporating the Charlson Co-Morbidity Index. “E” stands for the number of Emergency Department visits within the last 6 months.
Clinical Opiate Withdrawal Scale (COWS)
The Clinical Opiate Withdrawal Scale (COWS) is an 11-item scale designed to be administered by a clinician. This tool can be used in both inpatient and outpatient settings to reproducibly rate common signs and symptoms of opiate withdrawal and monitor these symptoms over time.
