New Ocean Forms – June 2015

Recent Additions to the Ocean Library

3 New Agenda-Setting Forms

Ocean clinics are increasingly using Ocean forms to manage the agenda-setting process and set patient expectations around the number of issues that can be addressed during a single appointment. These forms can be displayed on tablets in the waiting room, or sent to the patient in advance of the visit. The following three new forms cover basic agenda-setting, agenda-setting for a 15-minute appointment with a warning if more than two issues are raised, and agenda-setting for a 15-minute appointment where a follow-up appointment is required for more than one issue.

Antenatal Record 1 – Medical History Questions

This form covers the medical history questions required for the Antenatal Record 1.

Cocaine Abuse

This form presents sensitive questions around possible cocaine abuse.

Follow-up from Previous Visit

This basic pre-visit questionnaire for follow-up appointments helps to set the agenda and identify concerns since the previous appointment.

In addition, it assesses symptoms related to school phobias.
