Allergy Consultation Questionnaire
This is an allergy and immunology referral questionnaire with a full history for patients with allergies. This questionnaire is designed to automatically generate the history portion of a consult letter for allergists.
New Patient Registration – Pediatric
This intake questionnaire is an intake form to register new pediatric patients. It makes it easy for parents to quickly provide demographics and an initial basic history using a touchscreen tablet in the waiting room.

PTSD Checklist – Civilian Version
The PTSD Check List, Civilian Version (PCL-C) was developed by Frank Weathers and his colleagues at the National Centre for PTSD (1993). The scale consists of 17 questions that now correspond to DSM-IV.
PHQ-9 (Patient Health Questionnaire) for Low Mood
The Patient Health Questionnaire (PHQ) is a multiple-choice self-report inventory that is used as a screening and diagnostic tool for mental health disorders. The PHQ-9 is a mood/depression assessment tool that scores each of the 9 DSM-IV criteria based on the mood module from the original PRIME-MD.
Rivermead Post-Concussion Symptoms Questionnaire
This standardized and validated questionnaire assesses the scope and severity of post-concussion symptoms following a mild traumatic brain injury (MTBI) using a calculated point score. It is used to quantify and monitor the patient’s recovery.
